Patients’ Responsibilities: Patients are responsible for providing a complete and accurate medical history and for following the treatment plans and advice provided by their healthcare provider. Financial Agreement: I agree that I am responsible for all charges for services provided to me, my spouse, and my dependents, payable in full on the day services are rendered. I understand that Pacific Cosmetic and Facelift Center and Dr. Faraz Valaie do not file or bill any insurance.
